Our OMNI Relief Loan is designed to provide quick and affordable access to funds when they are most needed. Disclosures

Credit Report Authorization: By submitting this request, you authorize the Credit Union to obtain credit reports in connection with this request for credit and for any update, increase, renewal, extension, or collection of the credit received and for other accounts, products, or services we may offer you or for which you may qualify. You understand that the Credit Union will rely on information in this request and your credit report to make its decision. When applicable, spousal information may be requested as a part of this request. If you request, the Credit Union will tell you the name and address of any credit bureau from which it received a credit report on you.

Accuracy of Information: It is a crime to willfully and deliberately provide incomplete or incorrect information in this request.

Offer available only to individuals who live or work in the following Michigan counties: Branch, Cass, or St. Joseph County. Credit union membership eligibility is required. Loan amounts range from $500 to $5,000 with a fixed 36-month term; no other terms are available. Payments are due on the 20th of each month beginning the month following loan closing. Annual Percentage Rate (APR) is 3.99% for all approved applicants. All loans are subject to credit and underwriting approval. Proof of income and documentation of disaster-related impact may be required. This loan program is intended for individuals affected by the severe weather and tornadoes that occurred on March 6, 2026. Offer may be modified or discontinued at any time without notice.

You promise to pay the credit union the amount selected above plus interest on the unpaid balance at
the rate you have selected according to the schedule in the Truth-In-Lending Disclosure.

Notice to Co-Borrower: If you are signing this note as a co-borrower you agree to
be equally responsible with the borrower, and the credit union may collect the entire amount due
from you. The credit union does not have to notify you that this NOTE has been paid. The credit
union may delay enforcing any of their rights under this note without losing them.

Prepayment: You can prepay all or any part of this loan at any time without
penalty.

Late Charges: Fines for charges may be imposed for late payments on this loan up to
the maximum shown in the "Truth-In-Lending Disclosure."

Default: You are in default if you fail to make a payment by the scheduled due
date.

Acceleration: If you are in default, any part of this loan then unpaid is due and
payable without notice to you at the credit union's option. You agree to pay all usual and customary
costs of collection including reasonable attorney fees and all taxable court costs permitted by law.

No Waiver: The credit union can delay enforcing any of its rights any number of
times without losing its rights.
If payments on this loan are up to date, the borrower(s) can withdraw shares and/or deposits in
excess of $5. Your signature is your signature on the Note and Pledge of Savings. The borrower(s)
acknowledge receipt of this form prior to signing. Subject to credit approval.
